Check the "date" checkbox on TB reports

Description

The meaning of "date checkbox" on TB report 10, 11 and 12 aren't clear.

Check the real reason why those checkboxes are there and check if those checkboxes are beeing used on the query that generates the response.

TB12
uses registration date to filter cases

TB11
Uses registration date to filter the cases that are going to be counted.
If the user checks the "Start treatment date checkbox" the system filters the cases by the registration date and initial treatment date too.

TB10
Uses initial treatment date to filter the cases that are going to be counted.
If the user checks the "registration date checkbox" the system filters the cases by the initial treatment date and registration date too.

After a conference call with Gustavo the decisions was: show both check boxes for all reports (so the user will know that the report is using these filters), but in TB 11 and TB 12 both has to be checked and read-only, in TB 10 only the registration date check box has to be checked and read-only.
