Quantification tool

Based on the new approach (released in version 1.0.0.6):

- Keep the fields Name and Composition;

- Remove the field "Duration" in weeks;

- Keep the field "Type of Regimen", but with Multi Drug and Single Drug options. Remove Patient kits option from the dropdown.

- Keep the columns Default dose unit and Default weekly frequency;

- Remove the columns "From Week" and "To Week";

- Include the column "Duration (months)" replacing the "From Week and To Week" columns;

- When select Multi Drug in the "Type of Regimen", system display both Intensive Phase and Continuation Phase tables and users can add more then one medicine per phase;

- When select Single Drug in the "Type of Regimen", system hide the Intensive Phase label and also the entire Continuation Phase table;

- When select Single Drug in the "Type of Regimen" and after adding one medicine, system will disable the "Add Medicine" button, meaning the users can add only one medicine and not related with intensive or continuation phases.
