Description

- Turn the current columns into fields with dropdown (same approach as MSH Demo) and allow entering same effect more than one time;

- Display the fields in the following sort: Adverse Reaction; Start Date; Judged related to; Grading; Seriousness; Status Date; Action Taken; Outcome; Comments;

- When entering the same adverse effect the fields "Start Date" and "Judged related to" will display previous recorded data, but allowing users to change;

- System must generate new report (see form attached).
