Name of the person performing the forecasting, as a free text field;

Instructions for completing (i.e., listed or a link) or help (i.e., when hover the mouse);

The name of the country and person should be displayed in the result/tables of the forecasting and when it's saved or printed as well.

Please, accommodate the entire word "forecasting", avoiding the "forecasti..."

Lay-out on screen is okay. But when exporting to excel should be included the label of the fields before the name of country and person. See figure attached.

Display those information in two separate rows on the excel file.
