Description

Contact evaluation form - Fields “Eligible form INH”, “Commenced on INH”, and “Screened for TB” must have an empty option, which is the default when the contact is being registered. This fields must also be required, i.e, must enter a value Yes or No.

Contact evaluation form - Age field - Must accept just numbers;


Contact evaluation form - New field - Start date of INH - This field should be available only if ‘Commenced on INH” is Yes.
