[Tasks from Mauricio TO-DO List] Include explanation (like a wiki) for the auto-calculated fields

Description
The auto calculated fields need to have an explanation to the user explaining the rule that fill in that field.
It should be inserted an "?" after the field value that when the user puts the mouse over it, a window with the explanation texts appears.
The auto-calculated fields that exists now are:
- Treatment Category
- Second Line Drugs Received
- Follow up Smear Status (Bangladesh)
