Description

- Create new group of variables called "Adverse Effects".

- Include the existing fields (Adverse Reaction, Grading, Seriousness, Judged related to, Action Taken, Outcome) as sub-variables.

- Include the existing fields Start Date and Status Date as sub-variables in combination with the others.

- Include another sub-variable called "Frequency of episodes" with sub-options: one, two, three, four and more than four, in combination with the others.
